This page contains the application and required documents to apply for an underground mine license in Kentucky. The Mine licensee checklist may assist to ensure all the required parts of the application and accompanying documentation are completed and included with a submittal to the Divsion of Mine Safety (DMS).
Mine License Application Required application for a new, renewal, or company/operator changes.
Officers and Directors Page Required if the licensee is not an individual.
License fee Required to include one certified check or money order payable to the Kentucky State Treasurer in the amount of $300. Renewals are based on the tonnage from the previous year ($300 plus $100 for each additional 100,000 tons up to $1,500 maximum).
Application for Proposed Mine Openings Required only for a new mine opening or additional openings.
Emergency Action Plan & Certification Required for underground mines.
Ventilation Plan Certification Required along with a copy of an MSHA approved ventilation plan.
Fan Stoppage Plan Certification Required for underground mines.
Roof Control Plan Certification Required certification. A copy of a Roof Control Plan is available at one of our District Offices.
Solid Blasting Permit Application Required when solid mine blasting.
Diesel Powered Equipment Required prior to using diesel powered equipment in an underground mine.
Licensee Signature Authorization Form Required if the executive officer is unable to be present to sign all the required forms.
Certified maps - two (2) up-to-date mine maps both certified by a P.E. licensed in Kentucky and notarized. The maps are required to show a 12-month projection of the mine pursuant to KRS 352.450. In addition, any changes of a company or operator must also have two (2) final maps from the previous company or operator.
Map Transmittal Letter Required to accompany the mine maps.
Workers' Compensation coverage - DMS verifies coverage through the Kentucky Department of Labor.
Approval for a mine license from the Kentucky Department of Revenue.
